HOIST & CRANE OPERATOR’S DAILY CHECKLIST
INSTRUCTIONS: At the start of each shift, inspect or test the components of the hoist and/or crane noted below. Add specific comments and notify a manager immediately if any line is marked REJECT.

STEPS FOR SAFE LIFTING

ASSURE THE TOTAL LOAD TO BE LIFTED (INCLUDING RIGGING) DOES NOT EXCEED THE RATED CAPACITY OF THE HOIST.

OPERATE ALL CONTROLS TO INSURE THEIR PROPER OPERATION. DO NOT USE A DAMAGED OR MALFUNCTIONING HOIST.

TEST THE LIMIT SWITCH BY FIRST RAISING THE HOOK BLOCK SLOWLY AND THEN AT FULL SPEED. IF IT DOES NOT STOP THE HOISTING MOTION, STOP THE UPWARD MOVEMENTS AND REPORT FAILURE TO YOUR SUPERVISOR.

INSPECT THE HOOK, CHAIN AND WIRE ROPE. ASSURE NO SLACK IS PRESENT ON DRUM OR LOAD BLOCK.

ATTACH THE LOAD TO THE LOAD BLOCK HOOK BY MEANS OF SLINGS OR OTHER APPROVED DEVICES. ASSURE ITS PROPERLY SEATED IN THE SADDLE OF THE HOOK.

ASSURE THERE IS NO LOOSE GEAR OR MATERIAL ON THE LOAD.

IF NECESSARY, ATTACH TAG LINES TO CONTROL THE LOAD.

ASSURE THAT THE RIGGING AND LOAD WILL CLEAR ALL OBSTACLES AND CLEAR ALL NON-ESSENTIAL PERSONNEL FROM THE OPERATIONAL AREA.

RAISE THE LOAD A FEW INCHES TO VERIFY BALANCE AND EFFECTIVENESS OF THE BRAKING SYSTEM. IF THE EQUIPMENT OPERATES AS EXPECTED, PROCEED WITH THE LIFT.

KEEP THE LOAD OR BLOCK HIGH ENOUGH TO CLEAR ALL APPARATUS BELOW THE HOIST

LOWER THE LOAD CAREFULLY. KEEP THE LOAD UNDER CONTROL UNTIL IT IS SECURED.
- RETURN THE HOISTING DEVICES TO ITS DESIGNATED PARK POSITION AND OPEN THE MAIN CIRCUIT BREAKER.

DO NOT OPERATE HOIST WITH TWISTED, KINKED OR DAMAGED CHAIN OR ROPE

DO NOT OPERATE HOIST WITH OTHER THAN ITS OWN POWER

ALWAYS POSITION THE HOIST AND LOAD IN A DIRECT VERTICAL LINE (NO SIDE LOADING)
- DO NOT TRANSPORT LOADS OVER PERSONNEL
- DO NOT LEAVE THE CONTROLS WHILE A LOAD IS ON THE HOOK
- THE OPERATOR SHALL RESPOND TO SIGNALS ONLY FROM THE APPOINTED SIGNALER BUT MUST OBEY A STOP SIGNAL AT ALL TIMES, NO MATTER WHO GIVES IT
